Food Safety Forms & Checklists

Helpful information about the below forms/checklists?

1. Food Safety Monthly Maintenance = This form is to be used by the Food oversight or his/her delegated member responsible for managing a food premise.  (Due last Sunday of the month)

2. Routine Food Activities = Routine food activities consist of the following; a café, a food storehouse, a supermarket, a kitchen that’s used to prepare and store food for sale or for free.

3. Temporary Food Activities = Use this form to inform Safe Church of a temporary food event or a food activity organised to supplement your ministry.

4. Food Safety Process Control Form = Process Control Form (Comma, Kitchens, Catering)

5. External Food Vendors Application = Is the form External food vendors must fill out and submit 5 working days prior to an event.  External food vendor are businesses who are providing catering, food trucks/stalls retail on site at a Hillsong church.  We usually use them during food festivals and adhoc events.

Food Safety Training

Food Safety Training is available to Provide you with an induction into our food safety requirements and make it easier for you to meet safety and legal obligations under the relevant federal, state and local requirements for food safety training.

 

To obtain a report of food safety induction status for workers at your location.  A staff member can request this information for their location by email [email protected] and requesting a report.

Food Safety FAQs

What steps should I follow when running a temporary/Routine food activity (includes Sisterhood, Men's event, providing food for volunteers/members)?

 

  1. Complete Temporary/Routine Food Activities form [LINK]
  2. Ensure you have a food safety supervisor (FSS)? If not you can get team trained through accredited providers.  Note an FFS are required for all food activities conducted at your location.
  3. Ensure all team involved in food service must do the following:
    1. Safe Church Food safety training course [LINK] and adhere to the direction of FSS.
    2. Local FSS must provide each team member with an induction to the food area and operations.
    3. FSS completes the ‘Process Control Form’, MUST complete and submit form by end of the shift [LINK].
    4. Read and adhere to food safety signage and Food safety procedures [LINK].
    5. Report incidents [LINK]

Now action anything that needs to be done now. E.g., ensure that the team are meeting the Hillsong food safety plan requirements.

What is required to operate a food festival at your location (usually ran during Christmas spec or Easter)?

When organising a food festival at your location you required to complete, implement and manage the following.

  1. Conduct a risk assessment in Risk Ezi using template FES02 | FOOD FESTIVALS FOOD STALLS/TRUCKS TEMPLATE [LINK to Risk Ezi] Login required.
  2. Request following documents from the Vendor:
    • Food Safety Supervisor Certificate or Certificate of Attainment with successful completion of the following units of competency.If you are in NSW only send through your NSW Food Authority issued Food Safety Supervisor Certificate:
      • SITXFSA001 Use hygienic practices for food safety
      • SITXFSA002 Participate in safe food handling practices
    • A current copy of your Certificate of Currency for Product and Public Insurance Liability Cover with us stated as the Interested Party.
    • ABN Certificate or number
    • Permit or License from the NSW Food Authority or Local Council and a current copy of your most recent Council Inspection, whichever is relevant. The document you submit must reflect the registration number of the mobile cart, trailer, van or truck. The Food Vendor is responsible for ensuring that he/she has the relevant permit to trade in the Event location Council jurisdiction.
    • If any food is partially prepared or fully prepared at another site, forward a copy of your food safety plan.
    • Provide a copy of the Food Truck or Trailer or Van or Cart’s Gas and Electrical Compliance Certificate.

Once you’ve gathered all the documents, please use the checklist Approved Food Vendor Checklist [LINK] to check that you have satisfactorily onboarded your food vendors. Forward copies to [email protected] to add these vendors to the approved vendor directory.

 

  1. Provide the Vendor with Hillsong Australia Food Safety Requirements (below):
    • Agree to not use children in food preparation and sales area
    • All electrical equipment used is tagged and tested and al tests dates are current
    • Obtained all food products from reputable suppliers and maintained invoices for traceability purposes
    • Will transport, handle and store food in hygienic and sanitary conditions to ensure food is safe
    • An accurate probe-type thermometer will be used to maintain correct temperature
    • If potentially hazardous foods are stored between 5°C and 60°C, the length of time food is stored in this temperature will be tracked and records maintained.
    • Confirmed that all food handlers have skills and knowledge in hygiene & food safety matters.
    • If cooking, all foods will be thoroughly cooked to prevent contamination.
    • Where there is a potential risk of cross-contamination with food allergens, advise to NOT consume the food will be given to allergenic customers. Pre-packaged food will be clearly labelled to Australian & NZ Food Labelling Standards.
    • If requested, be able to provide a list of ingredients.
    • If food is going to be prepared externally and transported to events site, vendor is required to attach their food safety plan.

What to do when using a food vendor (e.g. food truck operator)
  1. The Hillsong team/location organising event complete ‘Temporary food activity form’ [LINK]
  2. If Food Vendors ( External: food trucks/stall holders) forward the following link to ‘External Food Vendor Form’ [LINK]. There is a message on this form that advising food vendors This form is required to be completed and required documents are supplied at least 5 working days prior to event for review and approval.  The event co-ordinator is responsible for ensuring external vendors complete and return the External food vendors form on time
  3. Once food vendors have completed the form a notification will be sent to campus leadership and events fore review file for current and future events.

What steps do I need to follow to get my location Council Approved?

When applying for a Council food permit there is a few steps and each step requires preparation.

Please follow the below steps below

1. Lodge an enquiry with your council

1.1 DO FIRST | Prepare for the enquiry:

Gather sufficient information: e.g., what food is being served (is it prepacked, requires preparation, ETC), days of operation, frequency of food activity weekly/monthly/once off, where preparation of food is taking place, prepare drawings of food stall/café/food truck ETC,

Ensure you have a food safety supervisor (FSS)? If not you can get team trained through accredited providers.  Note an FFS are required for all food activities conducted at your location.

Understand your compliance obligations. All food activities must adhere to:

How does a location change their Council Classification?

When applying for a change of Council Classification you must apply to Council and be approved (by council) before commencing any new food activity outside your permits classification.  Please follow the below steps to ensure your application is processed in a timely manner.  NOTE the process may take longer and cost more than anticipated, ensure you plan accordingly.

  1. Prepare to make enquiry

1.1 Locate current Council permits for your location.  If copies were provided to Safe Church Hillsong staff can locate this information in SC Document library [LINK].  If not please reach out to your local Leadership team and if they do not have it reach directly out to your local Council.

1.2 Ensure you have a food safety supervisor (FSS)?  If not you can get team trained through accredited providers.  Note an FFS are required for all food activities conducted at your location.

How to prepare for Council inspection?

To prepare for Council inspection conduct a self-assessment [LINK to checklist].  Follow any direction given from council.  Familiarise yourself with Food safety program link below.

Some key tasks to follow in addition to above self-assessment:

Ensure that all waste bins are lined with fitting lids.

Ensure that there are no gaps or holes where mice and pests can enter. Especially near the door, check to ensure no light is coming through (the bottom of the door). If you see light coming through this will mean you need to have pest strips. They sell these at bunnings.

Ensure all food safety signs and Council permit are displayed in the kitchen.  Laminate your copy and display neatly.

First aid kit in the kitchen to have blue band aids please.

Kindly request the person (volunteer/staff) who will be managing food safety to send you a photo of their Food Safety Supervisor qualifications or Certificate of Attainment.  Once you have this send me a copy please and keep a copy in the kitchen.  Laminate your copy and display neatly.

Do we require a qualified Food Safety Supervisor for our food activity?

What are the requirements for CityCare food operations?

If your CityCare activity involves the service of food?  You must ensure you have permission from your CityCare team manager and meet council requirements for the food activity.

Tip: For most local councils are pre-packed food, tea & coffee and sausage sizzle (sausage, bread, onions and sauce) are considered low risk, but you must confirm with the council in the area and follow their direction.  If you are providing food at a private venue like PCYC/Westfield you must adhere to the venues requirements for food service.

What training do volunteers require to serve in a food activity?

In addition to having a qualified  Food Safety Supervisor (FSS) All other team involved in food service must do the following:

    1. Safe Church Food safety training course [LINK] and adhere to the direction of FSS.
    2. Local FSS must provide each team member with an induction to the food area they are working/serving in.
What if someone get sick from eating food served from a food activity?
  1. If the incident is life threatening or an emergency situation call 000 Immediately
  2. Make sure the person is ok apply first aid if required and if pastoral care support is needed please liaise with management team
